Step 1: Call Us At 805-541-6000

Please ensure you have the following:

  • Insurance card (we will need an image of the front and back of your card)
  • Referring doctor / PCP / therapist info
  • Medication list (including dosage)
  • Preferred pharmacy and lab locations
  • Info regarding any psychiatric hospitalizations (facility name and length of stay)

During this intake call, our intake staff will complete an intake screening with you over the phone, gathering the pertinent information to indicate whether or not our clinicians are the best fit to provide care for you. This screening will take around 20 minutes.

You will be asked if you prefer in-person or telepsychiatry services, and will be matched to a clinician based on your preferences.

Step 2: Complete Intake Forms

If it is determined that we are an appropriate fit for your care, our intake staff will create your Athenahealth patient portal account and email you two packets of intake forms (administrative and clinical) from FormDr (our HIPAA compliant digital form software) at service@formdoctor-mail.com. These forms must be completely filled out and submitted digitally prior to scheduling your initial appointments.

Step 3: Schedule Your First Two Appointments

Once you have completed & submitted your forms, our intake staff will schedule your first two intake appointments. If you would like to be placed on a waitlist for an earlier appointment, please let us know.

What to Expect on Your First Appointment

Your treatment begins with an initial Psychiatric Evaluation, which will be performed over two appointments/sessions. The initial appointment will be 50 minutes. The goal is to come to an understanding of your specific psychiatric issues and develop a treatment plan.

The follow-up appointment will be 30-50 minutes, depending on the number and complexity of the issues being evaluated. Subsequent appointments vary in length from ten to fifty minutes, depending on the service provided.

How often you may need to be seen after these initial appointments will depend on your unique case. If you are starting a new medication, you may need to be seen every 1–2 weeks. Once your treatment plan is established and determined to be low in complexity, you may only need to be seen monthly.

Please note that for CCPN we only take senior plans and your insurance will require us to have an authorization. Non-senior plans for CCPN are billed under your home plan (such as Blue Cross or Blue Shield). If your insurance plan’s mental health benefit is administered by a third party, we will determine if we are in network upon receiving a copy of your insurance card.

Payment plans available: If you wish to set up a cash payment plan for our psychiatric services instead of using insurance, talk about this option with our intake specialists.
